Job Summary And Qualifications
- Assists in communicating to patient and/or provider, results of lung cancer screening, tissue sampling, etc.
- Attend patient care planning conference and other meetings as necessary.
- Ensure that appropriate patient data are available and documented at multidisciplinary meetings held to advise on clinical care and next steps.
- Document and complete actionable information resulting from physician consultation, nodule clinics and multi disciplinary meetings on behalf of the patient and clinician.
- Track and document interventions and outcomes.
- If appropriate, support definition of datasets and ensure appropriate data are collected to track program interventions and outcomes.
- Ensure appropriate and timely communication of patient progress to referring physicians and other care providers.
- Collaborate with program stakeholders in support of data collection for registry participation, quality reporting, etc.
- Drive process improvement.
- Collaborate and manage performance improvement activities as indicated by outcomes in program efficiency, patient experience and outcomes.
- Assist in changes to the current program both locally and at a corporate level to support ongoing improvement of the program model.
- Conduct outreach to referrers, providers and other medical professionals.
- Establish and maintain positive working relationships with key internal and external customers (e.g. physicians, nurses, radiology, social services, radiation oncology, business office, shared services, etc.).
- Educate each constituent on the role and benefits of the Nodule program and high quality cancer care.
- Recognize scope and limitations of role and regularly access clinical supervision as a support to the role.
- Stay current on the growing evidence and guidelines associated with lung cancer screening and incidental nodule follow-up.
- Connect patient and clinician as necessary to discuss risks and benefits of screening.
- Ensure service continuity.
- Establish appropriate mechanisms to ensure service continuity during planned and/or, unplanned absence(s).
- Meet regularly with cancer coordinators to ensure a seamless transition in care coordination.
- Other duties as assigned (e.g. program development, stakeholder training, data aggregation, business plan support, etc.)

- Review and understand medical records and other patient care information.
- Skills in computer systems including Microsoft Office, Outlook email and internet to support coordinator role.
- Effectively and appropriately, communicate (oral & written) objectives, goals, needs, and barriers with a variety of clinical and non-clinical stakeholders necessary to build confidence in the nodule program.
- Tenacity, self-motivation and initiative necessary to drive patient results and outcomes expected in the HCA Healthcare, Nodule program.
