Job Summary and Qualifications
CASE MANAGEMENT SOCIAL WORKER MSW – PRN
The Case Management Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.
What you will do in this role:
- Acts as a liaison between the patient and community resources/post-acute care providers
- Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care
- Assists the Case Manager RN in facilitating patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput
What qualifications you will need:
- Master of Social Work (MSW) required
- Licensed Masters Social Worker (LMSW) or License Clinical Social Worker (LCSW) required
- Certification in social work or case management preferred
- Acute care social work experience required
This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
