Job Summary And Qualifications
As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. The goal is to give an unmatched patient experience and attain optimal clinical outcomes.
What will you do in this role:
- Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
- Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
- Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.
- Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
- Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
- Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
- Demonstrates a time-conscious awareness and ability to prioritize. Distinguishes between low and high-priority activities.
- Associate Degree
- (APRN) Advanced Practice Registered Nurse, or (RN) Registered Nurse
- Current Florida license as Registered Nurse or Recognized Compact Licensure (subject to Florida State Licensing Requirements, including not limited to ongoing eligibility and duration provisions)
- Graduate from an accredited School of Nursing
- Current Basic Cardiac Life Support (BLS) certification or must be obtained within 30 days of employment start date
