This Role works as a mediator between clinical teams, patients, families, and next-level care providers to ensure seamless and comprehensive care.
The Mental Health Professional II facilitates high quality, patient-centered care by providing psychiatric treatment services to patients and their families. The Mental Health Professional II performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy.

The Mental Health Professional II performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy.
What you will do in this role:
- You will assess, plan, intervene and evaluate the care of psychiatric patients.
- You will instruct patients and families regarding treatment instructions
- Demonstrates advanced clinical skills and ability to plan and facilitate individual, group and family psychoeducational and process groups.
- Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.
- Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge. Provides guidance on complex issues.
- Conducts post-discharge follow-up as indicated.
- Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.
- Master’s degree in a Human Services related field (Psychology, Social Work, Mental Health, or Counseling
- Advanced educational preparation or experience in the area of clinical practice and/or management
- American Heart Association BLS (within 30 days of hire)
- Crisis Intervention Prevention (CPI) training required (within 30 days of hire)
- Conducts individual, group, and family psychoeducational groups.
- Contributes to treatment plan development and attends treatment planning meetings and reviews on an ongoing basis.
- Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
- Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
- Completes relevant assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
- Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
- Conducts post-discharge follow-up as indicated.
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
