Job Summary And Qualifications
The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.

Job Summary And Qualifications
The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.
Major Responsibilities:
- Coordinates with case manager and social worker for support needed
- Provides clerical support in multidisciplinary rounds
- Makes referrals through the web based referral system to the specific post-acute care (PAC) providers requested
- Monitors patients with PAC referrals and intervenes to expedite discharge
- Prioritizes workload to ensure a timely contact and intervention PAC providers
- Delivers the Beneficiary Notice to patients identified as being in the BPCI-A program
- Obtains physician signatures on required Case Management documents and forms
- Based on family practice; sets up transportation/send information to Transfer Center and generates discharge packets for PAC transfers
- Practices and adheres to the "Code of Conduct" and "Mission and Value Statement".
- High School Graduate / GED
- Associate degree preferred
- Medical knowledge and terminology preferred
- CNA, EMT preferred
- Minimum of 1 year experience in a healthcare setting
- Highschool Dipolma/GED
- CNA or Medical Assistant experience preferred
- Knowledge of medical terminology preferred
