Clinical Rehabilitation Liaison - PRN/Per Diem
We are currently looking for an ambitious Clinical Rehabilitation Liaison to join our on-site team on a PRN basis. As a Clinical Rehab Liaison, you are responsible for coordination of the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. You will act as a liaison between internal and external referral sources and internal operations. Clinical Rehabilitation Liaison - PRN/Per Diem

Your duties and responsibilities may include:
- Establish relationships with case managers, physicians, payers and other referral sources
- Ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehabilitation Medical Director
- Work closely with hospital Case Management to ensure a responsive and smooth referral flow and admission process for the acute-care, inpatient rehabilitation program
- Graduate of an accredited school in a clinical specialty
- 2 yrs. recent clinical care experience
- Experience in a liaison role and/or rehab sales is a plus
- Must be a Florida-licensed professional in a clinical healthcare specialty (RN or CCRN, Physical Therapist, Occupational Therapist, Speech Therapist, Respiratory Therapist)
- Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills
