Social WorkerSocial Worker Job Description:Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needsIdentifying individual social and emotional needsDeveloping and maintaining individualized care plansParticipating in the planning of the resident’s admission, community re-entry or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentWork with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stayWork with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communitySocial Worker Qualifications/Requirements:BSW or MSW preferred, or Bachelors degree in the related fieldAt least 3 years experience in the Social Work field requiredAbility to work flexible schedulePCC experience preferredSalary: Up to $80,000 a year (based on experience)An Equal Opportunity Employer
