We are seeking a compassionate and experienced Director of Social Services to lead our social services department and support the emotional, psychosocial, and discharge planning needs of our residents. This leadership role is ideal for a dedicated professional who is passionate about advocacy, regulatory compliance, and resident-centered care.
Position Summary
The Director of Social Services is responsible for overseeing all aspects of the social services program, ensuring residents and their families receive comprehensive support in accordance with federal, state, and facility regulations.
Director of Social Services Key Responsibilities
Conduct psychosocial assessments and develop individualized care plans
Oversee discharge planning and coordination of post-acute services
Provide counseling and support to residents and families
Ensure compliance with CMS and state regulatory requirements
Supervise and mentor social services staff
Participate in interdisciplinary team meetings and care conferences
Assist with admission processes and resident transitions
Address resident grievances and advocate for resident rights
Prepare for and participate in state and federal surveys
Maintain accurate documentation and department records
Director of Social Services Qualifications
Bachelor’s degree in Social Work required (Master’s preferred)
Current Social Work license (if required by state)
Minimum of 2–3 years of experience in a SNF or healthcare setting
Strong knowledge of long-term care regulations and discharge planning
Excellent communication and organizational skills
Ability to lead a team and manage multiple priorities
