Description
Health Navigators work within the Medical Suite and are responsible fpr providing health education, supporting patient needs, and lnking patients to appropriate resources. The individual will collaborate with staff, clients and teams to create personalized care plans, focusing on chronic medical conditions and healthcare navigation. Addditionally. they will assist patients with re-engaging in HIV care, following up on referrals and supporting PrEP screenings.
ESSENTIAL FUNCTIONS:
- Engage with patients through health education and support groups within medical units.
- Work collaboratively with other teams to identify client needs and help develop a plan to achieve health goals.
- Screen patientsfor PrEP eligibility, introduce services and support clinic staff with referrals.
- Meet with Patients to discuss referrals, process them and track through referral log.
- Manage patient referrals using the Referral Tracking Log ensuring accurate information entry.
- Introduce the PrEP program, screen patients, provide education and track participation
- Enter Data into PrEP tracking systems and assistwith appointment scheduling for interested patients.
- Enroll participants into 5 year project aimed at providing care for individuals living with substance use disorders and HIV/HCV.
- Conduct GPRA interviews, provide harm reduction materials and ensure data submission.
- Remain current on community resources & health services to assist patients in meeting their needs.
- Work closely with medical providers to coordinate patient care and schedule appointments with specialists.
Requirements
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
EDUCATION REQUIRED
- High school diploma/GED/or equivalent of 2 years work experience in social service setting required.
- Bachelors in social services or health related field preferred.
EXPERIENCE AND SKILLS REQUIRED:
- Knowledge of HIV, STIs, PrEP and Hepatitis C highly preferred.
- Experience working in health care and navigating health insurance is highly beneficial.
- Experience with Electronic Health Records preferred.
- At least 1 year previous experience working with substance users and knowledge around substance use and abuse required.
- Phlebotomy training preferred.
- Bilingual (English/Spanish) preferred
- Valid Illinois Driver’s License and eligibility for agency insurance coverage is preferred.
- Understanding of HIPPAA, 42CFR, AIDS Confidentiality Act.
- Previous case management experience beneficial.
- Previous counseling and testing experience beneficial.
