About the Position
Hawley Homecare is seeking an experienced, highly dependable, and solution-oriented Care Manager to oversee a portfolio of clients and the caregivers assigned to them.
This is not simply an administrative or scheduling position.
Our Care Managers are responsible for the overall success of their assigned clients. They serve as the primary relationship between Hawley Homecare, the client and family, and the caregivers providing services in the home.
We are looking for someone who can take ownership, solve problems, lead caregivers, communicate exceptionally well, and maintain high standards even when things do not go according to plan.
Home care is fast-paced and unpredictable. A caregiver may call off. A client's needs may suddenly change. A family member may have a concern. A schedule may need to be reorganized quickly.
The right Care Manager doesn't freeze under pressure or simply pass the problem to someone else. They assess the situation, communicate, find a solution, and follow through.
Who We're Looking For
Our ideal Care Manager is:
- An exceptional problem solver
- Dependable and accountable
- Highly organized
- Extremely attentive to detail
- Professional in appearance, communication, and conduct
- Calm and effective under pressure
- A strong communicator
- Comfortable making decisions
- Capable of managing several priorities simultaneously
- Confident coaching and holding caregivers accountable
- Compassionate toward clients and employees
- Proactive rather than reactive
- Able to exercise strong judgment and business acumen
- Someone who follows through and does what they say they will do
- Someone who takes genuine ownership of their responsibilities
We need someone who notices problems before they become bigger problems and takes action.
Client Management
Care Managers are responsible for maintaining excellent relationships with their assigned clients and ensuring services consistently meet Hawley Homecare's standards.
Responsibilities include:
- Serve as the primary point of contact for assigned clients and families.
- Ensure each client's established care plan is consistently followed.
- Conduct regular home visits with assigned clients.
- Maintain ongoing communication with clients and families regarding their services.
- Quickly investigate and resolve client concerns.
- Monitor client satisfaction and identify opportunities to improve services.
- Understand each client's routine, preferences, personality, and individual care needs.
- Participate in the onboarding and initial service period for new clients.
- Help ensure a smooth transition when new caregivers are introduced into a client's home.
Caregiver Leadership
Care Managers are also responsible for the performance and success of caregivers assigned to their clients.
Responsibilities include:
- Ensure caregivers understand and follow client care plans.
- Coach, train, and support caregivers when necessary.
- Ensure caregivers maintain professional appearance and proper company identification while working.
- Monitor caregiver attendance and punctuality.
- Ensure caregivers clock in and out correctly.

- Assist caregivers with the HCP application and other company technology.
- Train caregivers on the routines and expectations of newly assigned clients.
- Provide ongoing feedback and coaching.
- Participate in corrective action and disciplinary processes with management approval.
- Assist management with evaluating caregiver performance and staffing decisions.
Scheduling & Continuity of Care
One of the most important responsibilities of the Care Manager is helping ensure continuity of care for every assigned client.
When a caregiver becomes unavailable or calls off, the Care Manager is expected to take immediate ownership of the situation and work collaboratively with the scheduling and operations team to secure appropriate replacement coverage.
This includes:
- Identifying available and qualified replacement caregivers.
- Communicating promptly with scheduling, operations, and the client when appropriate.
- Reorganizing or adjusting schedules when necessary.
- Maintaining awareness of potential backup caregivers for assigned clients.
- Escalating unresolved staffing situations to the appropriate supervisor immediately.
- Remaining actively involved until an appropriate coverage plan has been established.
The expectation is ownership.
An uncovered shift should never simply be ignored, passed along without follow-up, or canceled without appropriate management involvement and authorization.
Operational Responsibilities
Care Managers will:
- Maintain sufficient staffing awareness for assigned clients.
- Review and adjust schedules when necessary.
- Communicate important client and caregiver developments to management.
- Keep supervisors informed of positive client feedback as well as concerns.
- Maintain accurate documentation.
- Follow established company policies and procedures.
- Participate in team and operational meetings.
- Work collaboratively with scheduling, recruiting, operations, and company leadership.
- Help identify recurring problems and recommend solutions.
- Maintain confidentiality and professionalism at all times.
Minimum Qualifications
- High school diploma or GED.
- At least 7 years of successful caregiving experience.
- Proven experience within non-medical home care.
- Strong understanding of the realities of providing care in a client's home.
- Excellent written and verbal communication skills.
- Strong organizational and time-management skills.
- Ability to work independently and make sound decisions.
- Ability to remain professional and productive during stressful situations.
- Comfortable learning and using scheduling systems, mobile applications, and office technology.
- Must successfully complete required background and reference checks.
- Reliable transportation and the ability to travel to client homes as required.

Preference may be given to candidates with:
- Certified Nursing Assistant (CNA) certification
- Medical Assistant (MA) certification
- Previous Care Manager experience
- Lead Caregiver or Field Supervisor experience
- Client Care Coordinator experience
- Home care scheduling experience
- Experience supervising or training caregivers
- Experience managing multiple home care clients simultaneously
Compensation
Base Salary: $43,000–$48,000 annually, depending on experience and qualifications.
In addition to base compensation, this position offers the opportunity to earn a generous performance-based bonus tied to successful management of assigned clients and operational performance.
Performance may include factors such as client satisfaction, client retention, staffing consistency, caregiver performance, service reliability, documentation compliance, and achievement of established operational goals.
We want high-performing Care Managers to have the opportunity to be financially rewarded for exceptional results.
Schedule
Monday–Friday, Full-Time. Occasional weekends.
Care Managers also participate in reasonable after-hours and weekend on-call responsibilities as appropriate.
Home care does not operate exclusively between 8:00 AM and 5:00 PM. Candidates must understand that occasional situations may require attention outside normal business hours.
At the same time, Hawley Homecare believes in developing systems, backup coverage, communication, and teamwork so that after-hours issues are managed effectively rather than unnecessarily falling on one individual.
The Standard
At Hawley Homecare, our clients trust us with something extremely important: their ability to safely and comfortably remain in their homes.
That responsibility requires exceptional people.
We are looking for a Care Manager who takes pride in their work, follows through, communicates well, notices the details, remains calm when challenges arise, and takes personal ownership of finding solutions.
If you are an experienced home care professional who thrives in a fast-paced environment and wants an opportunity where strong performance, leadership, and accountability are recognized and rewarded, we encourage you to apply.
