Position: Patient Navigator – Community & Social Health
Status: Full-Time, Non-Exempt
💵 Pay Range: Starting at $24.15/hour, commensurate with job skills and experience

You’ll work alongside a supportive interdisciplinary team in a value‑based care model designed to improve health outcomes, strengthen patient engagement, and reduce avoidable ER visits and hospitalizations. Translation? Your work truly matters.
✅ What We’re Looking For
Required:• High School Diploma or GED
• At least 2 years of experience in health or social services
• Strong knowledge of community resources
• Valid Hawaiʻi driver’s license, insurance, and reliable transportation
Preferred:• Bachelor’s degree in a human services‑related field
