Overview
Hiring an experienced RN with strong home health background and hands-on OASIS experience. Must be independent, detail-oriented, and comfortable with in-home patient care.
Responsibilities
- Perform Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge OASIS
- Conduct patient assessments and create care plans
- Coordinate with physicians and care team
- Monitor patient progress and update plans
- Educate patients and families
- Ensure Medicare compliance
- Complete documentation accurately and on time
Requirements
- Active RN license (Massachusetts)
- 1–2 years home health experience
- Strong OASIS experience (required)
- Knowledge of Medicare guidelines
- Valid driver’s license reliable transportation
Preferred
- Experience with Alora Plus
- EMR documentation experience
