Havencrest Rehabilitation and Healthcare Center is seeking an experienced Registered Nurse Assessment Coordinator (RNAC/MDS Coordinator) to join our clinical leadership team.
If you have strong MDS experience and are looking for a Monday–Friday position with a hybrid schedule, no on-call responsibilities, and no floor nursing duties, this could be the opportunity for you.

RNAC Responsibilities
- Complete and coordinate MDS assessments in accordance with CMS requirements
- Ensure accurate and timely completion and submission of MDS and RAI documentation
- Coordinate and participate in interdisciplinary care planning
- Review resident documentation for accuracy, completeness, and regulatory compliance
- Monitor documentation to support appropriate reimbursement
- Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members
- Maintain compliance with Medicare, Medicaid, CMS, and RAI guidelines
- Support quality measures and positive resident outcomes
Qualifications
- Current Pennsylvania Registered Nurse (RN) license in good standing
- Previous RNAC/MDS experience required
- Strong knowledge of MDS, RAI, Medicare, Medicaid, and reimbursement guidelines
- Excellent organizational and time-management skills
- Strong attention to detail and clinical documentation
- Effective communication and leadership skills
