Responsibilities
- Deliver comprehensive patient observation and monitor vital signs regularly to ensure safety and well-being.
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting/incontinence care, and mobility support, meal prep, cognitive supervision.
- Medications reminders and maintain detailed medical documentation in Electronic Health Record (EHR) systems.
- Provide specialized dementia and memory care, utilizing techniques tailored to individuals with this dementia disease or other cognitive impairments.
- Maintain compliance with HIPAA regulations by safeguarding patient privacy and confidentiality at all times.
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment.
- Document medical records accurately, including medication administration logs and vital signs reports.
- Proven experience in patient care within home health, assisted living, or nursing home environments.
- Strong knowledge of medical terminology, medication administration procedures, and vital signs monitoring.
- Expertise in dementia care techniques, Alzheimer’s care strategies, and memory support approaches.
- Skilled in using EMR (Electronic Medical Records) and EHR systems for accurate documentation.
- Ability to assist with ADLs efficiently while maintaining dignity and respect for clients’ independence.
- Experience working with individuals with developmental disabilities or disabilities requiring specialized attention.
- CPR certification.
- Familiarity with medical equipment such as Hoyer lifts and other mobility aids; capable of assisting with transfers safely.
- Excellent communication skills to collaborate effectively with healthcare teams, clients, and families.
