What are the responsibilities and job description for the RN Clinical Manager position at Haven Home Health?
The Clinical Manager coordinates and oversees direct care provided to patients by clinical personnel. Develops, plans, implements, analyzes, and organizes clinical operations for a specific group of patients. Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies. Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning.
This position requires someone with good time management skills. One that will foster a culture of teamwork and collaboration within the team. Dependability and good communication skills are a must.
