What You'll Do
- Support Care Delivery
- Provide clinical oversight and support to field clinicians within your scope
- Help manage caseloads, coordinate visits, and ensure care is aligned with patient needs and agency standards
- Facilitate regular clinical check-ins and participate in biweekly case conferences to review patient progress and promote team collaboration
- Develop as a Leader
- Serve as a point of contact for your care team, offering guidance and encouragement
- Participate in ongoing leadership development and mentorship through Haven’s clinician-focused training approach
- Help onboard and coach new team members, creating a strong and supportive team culture
- Drive Quality and Communication
- Work closely with intake, scheduling, and leadership teams to ensure smooth transitions and continuity of care
- Monitor clinical outcomes, documentation timelines, and team performance metrics
- Assist with quality improvement efforts, compliance monitoring, and patient satisfaction initiatives
- Licensed RN, PT, OT, or SLP in the state of Missouri
- At least 1 year of recent home health experience
- Confident communicator and collaborative team player
- Interested in growing into a leadership or mentorship role
- Passionate about delivering high-quality, patient-focused care
