Responsibilities
As a Hospice Aide, you will:
- Provide personal care including bathing, grooming, dressing, and hygiene assistance
- Assist patients with mobility, transfers, ambulation, and positioning
- Support patients with meal preparation and feeding as needed
- Assist with toileting and elimination needs
- Monitor and report changes in patient condition to the clinical team
- Record vital signs, intake/output, and other observations as directed
- Perform light housekeeping tasks related to patient care
- Maintain accurate and timely documentation of patient visits
- Participate in care conferences and team meetings as assigned
- Promote patient comfort, dignity, and safety at all times
Qualifications
- Current Oklahoma Certified Home Health Aide (CHHA) certification required
- Previous hospice, home health, CNA, or direct patient care experience preferred
- Valid driver's license
- Reliable transportation and current auto insurance
- Strong communication and interpersonal skills
- Ability to work independently in patient homes
- Compassionate, dependable, and patient-centered approach to care
Physical Requirements
- Ability to lift up to 50 pounds
- Ability to assist with patient transfers and mobility
- Ability to travel throughout the service area
