We are seeking a Speech Therapist to join our team in the Seattle, WA area. In this role, you will serve as a trusted partner to your patients, guiding them through their individualized plan of care. You’ll focus on restoring communication and swallowing function, enhancing safety in the home, and helping patients reach their highest level of independence. Working under the physician-directed Plan of Care and in collaboration with patients and caregivers, you’ll play a vital role in promoting recovery, improving quality of life, and supporting overall health.

Qualifications:

• Current Speech Therapist license in the State of WA with completed fingerprinting
• Two (2) years of home health experience preferred
• Commitment to the philosophy and goals of Haven Home Health Care
• Ability to exercise initiative, sound judgment, and independence

Responsibilities:

• Adhere to agency policies and procedures
• Provide skilled speech therapy interventions as ordered by the physician
• Conduct initial evaluations and histories
• Develop, implement, and revise individualized plans of care
• Document patient progress with accurate clinical notes
• Participate in interdisciplinary patient care planning and conferences
• Consult with physicians regarding treatment modifications
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition documentation within required timeframes
