Job Summary
Under the general direction of nursing leadership and with provider collaboration and guidance, the Registered Nurse utilizes evidence-based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Medical Group.
- Provides nursing care, primarily administrative, including but not limited to, patient education, communicating results and provider recommendations, referrals via phone and Epic.
- Coordinates and provides holistic patient centered care.
- Utilizes evidence-based practice in accordance with the department and/or medical group policies and procedures to ensure safe quality care and customer service.
- Incorporates the art, advocacy, science and ethics in nursing effectively in their daily practice.
- Demonstrates and actively supports research utilization through inquiring into evidence-based practices.
- Evaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and family.
- Identifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patients.
- This role requires coverage at regional offices bringing variety to your workday and the chance to collaborate across multiple teams and communities.
Education
- Graduate of an accredited School of Nursing.
- Associate’s degree in Nursing; BSN, preferred
- Successful completion of nurse residency, if applicable.
- One year of experience with a minimum of 1000 hours of clinical experience as a licensed registered nurse.
- Registered Nurse, State of CT
- Certified BLS Provider credentialed from the American Heart Association (AHA).
- Additional certifications may be required in specific specialties
