Duties
- Collaborate with healthcare professionals to coordinate patient care and ensure treatment goals are met
- Conduct comprehensive assessments of patients' physical conditions and functional abilities
- Develop individualized treatment plans tailored to each patient's needs and recovery objectives
- Implement therapeutic interventions, including exercises, manual therapy, and other modalities
- Monitor patient progress and adjust treatment strategies accordingly
- Educate patients and their families on therapy techniques, injury prevention, and health maintenance
- Maintain accurate documentation of patient assessments, treatment sessions, and progress notes in compliance with regulatory standards
- Provide home health services, ensuring safety and comfort during therapy sessions
- Adhere to all safety protocols and maintain CPR certification at the time of employment
Experience
- Graduate of an accredited Physical Therapy Program from an American Physical Therapy Association (APTA) recognized school, with a BS degree or entry-level master's degree
- Active Connecticut PT license is required
- CPR certification must be current at the start of employment
- Prior experience in home health settings is preferred but not mandatory; strong communication skills and a collaborative attitude are essential for success in this role

Pay: $74,276.80 - $133,723.20 per year
