Overview

Join our dynamic LTC team as an LPN Clinical Care Coordinator, where your expertise in nursing and patient care will drive exceptional outcomes and elevate the patient experience. In this vital role, you will coordinate clinical activities, support interdisciplinary teams, and ensure seamless delivery of high-quality care across various settings. Your proactive approach and clinical knowledge will empower patients on their journey to recovery and wellness, making a meaningful difference every day.

Scheduled hours are 8:00AM to 4:30PM Monday through Friday with optional overtime available.

Responsibilities

  • Conduct comprehensive patient assessments.
  • Manage patient intake processes and facilitate discharge planning to ensure smooth transitions between inpatient, outpatient, and post-acute care settings.
  • Collaborate with physicians, specialists, and multidisciplinary teams to develop and implement individualized care plans aligned with SNF protocols and patient needs.
  • Utilize electronic health records (EHR) systems to document medical records accurately, track patient progress, and manage medical documentation in compliance with HIPAA regulations.
  • Support SNF policies related to health regulation compliance, infection control protocols, and patient service standards to uphold safety and quality assurance.

Requirements

  • Valid Licensed Practical Nurse (LPN) license with current registration.
  • Proven critical care experience within hospital environments such as ICU, emergency room, or trauma centers.
  • Experience working in a SNF or related healthcare setting.
  • Strong knowledge of medical terminology, physiology, and hospital medicine practices.
  • Familiarity with electronic health records (EHR) management systems and medical records documentation standards.
  • Excellent patient assessment skills combined with the ability to manage multiple priorities in fast-paced settings like emergency medicine or outpatient clinics.
  • Knowledge of HIPAA regulations and healthcare policies related to infectious disease care, discharge planning, managed care, and utilization management.
  • Ability to work effectively within multidisciplinary teams across inpatient and LTC environments including primary care clinics.
  • Demonstrated commitment to patient service excellence and compassionate care delivery.
