ACCESS Harris County is an innovative, groundbreaking safety net care coordination delivery initiative designed to improve the health, well-being, sustained recovery, and self-sufficiency of the County’s most vulnerable residents. The objectives of the ACCESS Harris County Initiative are achieved through the integration and coordination of care across County safety net departments and community-based organizations who are currently providing services to residents in need of services across these systems but are currently accessing them in a siloed and fragmented manner.
The ACCESS Initiative focuses on shared populations in need of services and supports across the safety net continuum. Priority cohorts include:
Under the supervision of the ACCESS Harris IMDT Manager, the Violence Prevention Cohort Manager will oversee a team of coordinated care professionals to achieve the goals of the Violence Prevention cohort of ACCESS Harris. The Cohort Manager will be responsible for leading and supervising the coordinated care team, training new staff members, providing logistical guidance and support, maintaining a working knowledge of and compliance with ACCESS Harris objectives and procedures, and coordinating with additional supervisors to deliver superior client care. The Cohort Manager will also oversee outreach efforts to increase access, communication, community engagement, and education for residents of Harris County.

- Homeless individuals with co-occurring health and behavioral health conditions
- Re-entry populations
- Transitional Age Youth aging out of the foster care system

- Multidisciplinary care coordination teams consisting of frontline staff across safety net departments and community-based organizations that provide health, behavioral health, housing, social services, food assistance, workforce development and training, and other safety providers
- Portfolio of services and interventions
- Enabling technology that allows for data sharing through an integrated data hub and virtual client record

Responsibilities also include:
- Serves as Team Lead of the ACCESS Violence Prevention Program
- Demonstrates advanced knowledge, experience, and skill in Violence Prevention
- Leads and facilitates ACCESS Violence Prevention interdisciplinary team discussions
- Guides and supervises ACCESS Violence Prevention staff in care plan activities
- Actively participates on committees and task forces to meet program goals
- Assists in monitoring of data to assess the current state and identify areas needing improvement
- Supports Innovation
- Oversee operations/activities for cohort-specific interdepartmental multi-disciplinary team
- Be the main point of contact for ACCESS Harris IMDT Manager, care coordination team, and safety net department appointed contacts.
- Responsible for workflow facilitation that increases productivity and efficiency to meet ACCESS Harris program initiatives.
- Evaluate, document, and continuously improve team performance.
- Develops status reports, metrics, or ad hoc reports, as requested by leadership
- Create consistent outreach plans to provide education and awareness of ACCESS Harris.
- Provides oversight and direction to employees within the team by organizing, maintaining staffing levels, ensuring new employee training, performance management, etc.
- Develop and maintain knowledge of operations and functions of Harris County government associated with achieving the goals and vision of Harris County Public Health through ACCESS Harris and related services.
- Ensure compliance and conformance with internal policies, procedures, and protocols along with external guidelines, laws, and regulations.
- Other duties as assigned, including special tasks involved in responding to an emergency event

Education Requirements:
- Bachelor’s degree in Business Administration, Healthcare Administration, Public Health, Social Work, Nursing, or related field.
- Valid Driver’s License (TX upon hire)
- Two (2) years of experience in a supervisory capacity
- Experience leading teams to reach challenging goals and objectives in the social service space
- Proficient in Microsoft Office Suite software including 365 programs
- Excellent written and verbal communication skills.
- Working knowledge of best management practices
- Ability to effectively collaborate with a variety of stakeholders both internal and external.

Education Preferred:
- MSW, MSN, MHA, or related field of study
- Experience in supervisory capacity specifically in a healthcare or social service setting.
