Join our compassionate team as a Home Care Aide, where you will play a vital role in providing personalized, respectful, and high-quality care to individuals in their homes or community settings. This rewarding position allows you to make a meaningful difference in the lives of clients by supporting their daily activities, promoting independence, and ensuring their safety and comfort. Schedule: Mondays, Wednesdays and Fridays 7:30am-4:30pm. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and independence.
- Monitor patients’ health status by observing vital signs, assessing mental and physical condition, and reporting any changes to healthcare professionals promptly.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary.
- Support clients with mobility and transfers using proper techniques such as Hoyer lifts or patient transfer methods to ensure safety for both client and caregiver.
- Maintain accurate documentation of care plans, patient observations, medication administration, and other relevant records while adhering to HIPAA regulations for confidentiality.
- Provide companionship and emotional support to foster a positive environment that enhances mental well-being.
- Assist with medication reminders and basic nursing tasks under supervision, including first aid or CPR when needed.
- Support clients with community engagement by providing transportation assistance for appointments or errands.
- Follow all safety protocols related to manual handling, heavy lifting, and infection control practices.
Qualifications
- Prior experience working with individuals with disabilities, neurodevelopmental disorders such as autism or dementia care is highly preferred.
- Demonstrated experience in home health, assisted living, nursing home care, or geriatrics is advantageous.
- Knowledge of patient observation techniques, care plans, medication administration, and vital signs monitoring.
- Ability to perform manual handling tasks safely using appropriate equipment like Hoyer lifts or patient transfer methods.
- Strong communication skills for effective client interaction and documentation while maintaining confidentiality per HIPAA standards.
- Certification in CPR and First Aid is required; additional certifications related to hospice care or Alzheimer's/dementia care are a plus.
- Physical ability to perform heavy lifting and assist with mobility tasks consistently throughout your shift.
- Compassionate attitude with a genuine desire to support individuals with developmental disabilities or aging populations in their homes or community settings.
