About the Company
HarmonyCares is expanding into the South Central Pennsylvania market and is seeking a Physician Lead (Medical Pod Leader) to help launch and grow our home-based primary care practice. This is a unique opportunity - with up to 50% of your time dedicated to leadership responsibilities - to build a new practice with the support of an established, physician-led organization that has delivered home-based primary care for more than 30 years and now operates in 14 states nationwide. Our model is built on value-based care, allowing physicians to focus on quality outcomes and meaningful patient relationships rather than high patient volume. HarmonyCares physicians and APPs deliver primary care by visiting medically complex patients in their homes, working alongside a care team that helps coordinate services, manage chronic conditions, and support patients between visits. Our providers see a manageable number of patients each day (typically 6-9,) allowing time for thoughtful visits, care coordination, and strong relationships with patients and families.
