The MCD actively oversees resident safety, behavior expressions, routines, and support with activities of daily living (ADLs). In close collaboration with the Healthcare Director, the MCD provides leadership and direction to medication technicians and nursing assistants, supporting the delivery of compassionate, high-quality ca
re.
Responsibili
- ties
As the designated process owner of the Memory Care Points of Distinction, the MCD ensures the consistent implementation of the Dining with Dignity Program, daily engagement programming, and other core components that promote dignity, independence, and overall well-b - eing.This position must be held by a Registered Nurse (RN) or Licensed Practical Nurse (LPN), as required by state regulations, and must operate within the scope of practice defined by the applicable State Nurse Practice
- Act.Evaluates residents prior to move-in to determine whether their care and service needs can be met in the commu
- nity.Conducts re-evaluations as indicated by the residents’ condition and/or State regulat
- ions.Develops a service plan unique to each resident based on the evaluation and the resident’s stated prefere
- nces.Revises the service plan as indicated by the residents’ condition and/or State regulat
- ions.Collaborate with each resident’s physician, pharmacist, third party healthcare providers, and other members of the healthcare team to promote coordinated care and serv
- ices.Collaborates/participates in the oversight of the community’s Medication Management Oversight Pro
- gram.Collaborates with the phar
- macy.Conducts med tech training and skills validation, where applicable by state regulat
- ions.Performs routine audits and other activities that promote safety in medication administra
- tion.Maintains a working knowledge of all applicable local, State, and Federal regulat
- ions.Assures that resident evaluations and documentation meet all requirements and collaborates with the HCD to assure that the resident care department is survey ready at all t
- imes.Conducts interviews, orientation, coaching and ongoing training of Harmony Square team members in accordance with associate training standards and regulatory guidel
- ines.Provide supervision and direction to Harmony Square team members in collaboration with the Healthcare Director to ensure consistent, high-quality care and sup
- port.Collaborate with the Life Enrichment Director to ensure the delivery of purposeful daily programming within the community and through external outings and ev
- ents.Lead a minimum of 1 high quality, innovative program per m
- onth.Plan, lead, and facilitate monthly Family Council Meetings to foster open communication, share updates, and strengthen family engagement within the commu
- nity.Partner with Alzheimer’s Association or other local organizations to schedule, organize and participate in Support Group meetings mon
- thly.Utilize and connect with local community resou
- rces.Maintain Behavior Expression pro
- cess.Ensure all aspects of dining are being met through partnership with the Director of Dining and Healthcare Dire
- ctor.Completes daily and bi-weekly neighborhood ro

Qualifi
- cations
Current valid and unencumbered license to practice as an RN or LPN in the State in which the community is - located.Areas of Study:
- Nursing.Experience in caring for geriatric individuals and those with Alzheimer’s and other forms of dementia strongly pr
- eferred.1-2 years’ experience required; 3 or more years' experience pr
- eferred.Special Certificates: Current CPR certification r
- equired.Dementia training or Certified Dementia Practitioner preferred or eligible for certif
ication.
