Position Summary
We are seeking a Hospice/Palliative Social Worker to join our interdisciplinary team. This role is essential in providing emotional support, counseling, and resource coordination for patients and families navigating end-of-life care.
Key Responsibilities
- Provide psychosocial assessments and develop individualized care plans.
- Offer counseling and emotional support to patients and families.
- Assist with advance care planning and end-of-life decision-making.
- Coordinate community resources and referrals as needed.
- Collaborate with the hospice team to ensure holistic care.
Qualifications
- Master's degree in Social Work (MSW) required.
- Current state licensure as a Social Worker.
- Experience in hospice, palliative care, or healthcare setting preferred.
- Spanish-speaking skills strongly preferred.
- Excellent communication, empathy, and organizational skills.
