Role Description The Licensed Clinical Social Worker (LCSW) – Contract role is a remote position responsible for providing psychosocial assessments, counseling, and support to hospice and palliative care patients and their families. Daily tasks include conducting telehealth or phone-based evaluations, developing individualized care plans, and coordinating services with the interdisciplinary team. The LCSW offers crisis intervention, grief and bereavement support, and education on coping strategies, community resources, and advance care planning. This role involves regular collaboration with nurses, physicians, and other care team members to address mental health, behavioral health, and social needs that impact care. Documentation of assessments, interventions, and care outcomes in accordance with regulatory and organizational standards is an essential part of the contract position.

Qualifications

• Demonstrated expertise in Social Work and Medical Social Work in hospice, palliative care, or related healthcare settings.
• Experience in Case Management, including coordination of services, resource linkage, and advocacy for patients and families.
• Strong background in Mental Health and Behavioral Health, including counseling, crisis intervention, and support for complex psychosocial needs.
• Active, unrestricted Licensed Clinical Social Worker (LCSW) credential in the relevant state(s) of practice.
• Prior experience in hospice, palliative care, or serious illness care is strongly preferred.
• Proficiency in remote practice, including use of telehealth platforms, secure documentation systems, and professional communication via phone and video.
• Excellent interpersonal, communication, and cultural humility skills, with the ability to support individuals and families from diverse backgrounds.
• Ability to work independently, manage a flexible caseload, and meet documentation and regulatory requirements as a contract professional.
