The Role
- Provide compassionate hospice care to patients in their homes, facilities, and surrounding communities
- Support patients and families with professionalism, empathy, and consistency
- Complete visits, documentation, and follow-up in a timely manner
- Collaborate with an interdisciplinary hospice team that values communication and teamwork
- Assist with routine visits, PRN visits, admissions support, or on-call support depending on experience and agency needs
- Compassionate and patient-centered
- Reliable and accountable
- Organized and able to work independently
- Comfortable traveling throughout San Antonio and nearby surrounding areas
- Strong communicators with patients, families, and team members
- Committed to providing thoughtful and consistent hospice care
- Current Texas RN or LVN license in good standing
- Hospice experience preferred, but not required for the right candidate
- Current driver’s license and reliable transportation
- Ability to document accurately and professionally
- Flexible PRN scheduling
