What are the responsibilities and job description for the Hospice RN (Registered Nurse) position at Harmony Hospice (2)?
We are seeking full-time, part-time, PRN and on-call RNs to join our hospice team! The hospice RN provides skilled nursing care to hospice patients in accordance with physician orders, the interdisciplinary plan of care, and the hospice philosophy. The RN is responsible for coordinating patient care, managing symptoms, educating caregivers, and ensuring documentation supports regulatory compliance under Medicare Conditions of Participation.
Essential Duties and Responsibilities
Clinical Care
- Completes initial and ongoing comprehensive assessments of hospice patients.
- Develops, implements, and updates individualized plans of care in collaboration with the Interdisciplinary Group (IDG).
- Administers medications, treatments, and interventions as ordered.
- Provides pain and symptom management consistent with hospice standards.
- Monitors patient condition and reports changes to the physician and IDG.
- Provides education and support to patients and caregivers regarding disease progression and end-of-life care.
- Supervises and directs hospice aides in accordance with regulatory requirements.
Care Coordination
- Participates in IDG meetings
- Collaborates with physicians, nurse practitioners, social workers, chaplains, and other team members.
- Coordinates care across settings, including home, assisted living, skilled nursing facilities, and inpatient units.
- Assists with transitions in care when appropriate.
Regulatory Compliance & Documentation
- Ensures documentation is timely, accurate, and supports hospice eligibility and terminal prognosis.
- Completes visit notes, care plan updates, and assessments in accordance with agency policy and regulatory requirements.
- Ensures physician orders are obtained and implemented.
- Completes hospice aide supervisory visits within required timeframes.
- Adheres to infection control, safety, and emergency preparedness policies.
- Reports incidents, complaints, and adverse events per agency protocol.
On-Call Responsibilities
- Participates in on-call rotation as assigned.
- Responds to urgent patient needs after hours, weekends, and holidays as assigned
- Provides crisis intervention and symptom management as needed.
Qualifications
- Graduate of an accredited school of nursing.
- Current, unrestricted Registered Nurse (RN) license in Massachusetts.
- Minimum one (1) year of nursing experience required.
- Hospice, home health, oncology, geriatrics, or palliative care experience strongly preferred.
- Current CPR certification.
- Valid driver’s license and reliable transportation (if field-based).
- Knowledge of hospice philosophy and end-of-life care principles.
- Understanding of Medicare hospice eligibility criteria preferred.
- Strong clinical assessment and communication skills.
- Ability to work independently in patient homes and facility settings.
- Ability to maintain professional boundaries and compassionate care delivery.
- Ability to lift up to 50 pounds with assistance.
- Ability to stand, walk, bend, and assist patients with mobility as needed.
- Ability to travel to patient care locations.
