What are the responsibilities and job description for the Resident Care Coordinator position at Harmony Homes Assisted Living?
Purpose: The Resident Care Coordinator leads and oversees all resident care operations within the assisted living residence, ensuring residents receive high quality, person centered services that uphold dignity, independence, safety, and well-being. This role ensures compliance with federal, state, and local regulations; provides leadership and supervision to resident care staff; and supports a collaborative, supportive environment for residents, families, and employees.
Essential Responsibilities
1. Leadership & Operational Oversight
Ensure residence operations are in full compliance with all applicable federal, state, and local regulations as well as Harmony Homes policies. Model and promote resident-centered care aligned with dignity, privacy, independence, choice, and individuality. Provide strong leadership that supports teamwork, professionalism, and accountability. Maintain flexible hours and on-call availability to meet operational needs.
2. Staff Management & Supervision
Oversee hiring, orientation, coaching, evaluation, and corrective action for resident care staff. Manage staffing plans, workloads, and role assignments to support safe and effective operations. Coordinate required training and in-service education with HR. Develop, maintain, and post staff schedules; manage time-off requests and vacation planning. Complete bi-weekly payroll for all assigned staff. Track, trend and report on shift compliance & attendance.
3*. Resident Care Planning & Documentation*
Collaborate with the RN to develop individualized, up-to-date resident care plans that reflect each resident’s needs and preferences. Ensure care plans are reviewed regularly and revised following any change in condition. Oversee resident documentation including ADL records, shower logs/refusal notices, toileting schedules, and housekeeping lists. 4*. Resident Support, Admissions & Transitions*
Manage resident care documentation. Assist with resident care as needed to ensure high-quality service delivery.
5*. Family, Resident & Staff Communication *
Respond promptly and professionally to concerns or complaints expressed by residents, families, or staff. Conduct investigations and escalate issues appropriately. Maintain positive, supportive relationships with residents and families.
6. Regulatory Compliance & Reporting
Report all unusual or reportable incidents, including resident-to-resident incidents, in accordance with DHHS requirements. Report suspected abuse, neglect, or exploitation to the Ombudsman, Adult Protective Services, State Licensing, and responsible parties. Maintain compliance with all safety, infection control, and environmental care standards. Provide regular reports and updates to the Owner/Executive Director. Complete resident chart audits at least 2 times per year.
7. Administrative Delegate
Maintain the competence required to support the facility, staff, and residents and complete tasks in the temporary absence of the Facility Administrator.
Minimum Qualifications Education
- High school diploma or GED required.
- Additional coursework or certification in healthcare administration, gerontology, nursing, or human services preferred.
- CARES Dementia Training required (or obtained within 90 days of hire).
Experience
- Demonstrated leadership and supervisory experience in assisted living, long-term care, home care, or a related eldercare setting.
- Experience developing care plans, managing staff, and coordinating resident services is highly preferred.
- Experience working with older adults and strong understanding of dementia-informed care practices.
Qualifications
- Ability to plan, organize, develop, and implement programs and procedures.
- Strong verbal and written communication skills.
- Ability to relate professionally to the public, residents, families, staff, and outside professionals.
- Must be at least 21 years of age.
- Strong working knowledge of applicable regulations governing assisted living services.

Physical / Environmental Requirements
- Ability to walk throughout the assisted living residence for extended periods.
- Ability to lift, push, pull, or carry 25–30 lbs. occasionally.
- Ability to bend, reach, stoop, kneel, and perform repetitive motions as required.
- Ability to assist in emergency situations, including evacuations or resident transfers.
- Must work effectively in a fast-paced setting with frequent interruptions. Exposure to typical assisted living conditions including resident rooms, dining areas, and common spaces.
- Must adhere to infection control standards and use PPE as required
Pay: $28.00 - $38.00 per hour
