Position Summary
Harmony Health is seeking a Part-Time Caregiver (Direct Support Professional) to provide support for a male member in Hermitage, TN. This position is 14.5 hours per week, three times a week (M/W/F), start time after 8am/9am.
The member you would be supporting is wheelchair bound, alert, and oriented and requires help with ADLs (bed baths, laundry, cleaning his room, and driving to appoints - can use member's car). This member requires bed baths, but can transfer himself from wheelchair to bed and vice versa. This is a smoking home and this member has one dog who he describes as friendly.
Responsibilities / Job Duties
- Assist member with activities of daily living, including personal hygiene, dining skills, housekeeping, grocery shopping, social skills, and community access.
- Provide direct support and assist with individualized training activities as outlined in the member’s ISP (Individual Support Plan).
- Assist with meal preparation and promote social skills during meals.
- Monitor medications and treatments according to company policy.
- Support member in maintaining her home environment, including basic housekeeping.
- Ensure the safety, health, and wellbeing of the member at all times.
- Complete accurate documentation, observation notes, and daily reports.
- Attend and successfully complete required in-service training.
- Treat member with dignity and respect, promoting independence and choice.
- Provide transportation and mobility support as needed.
Requirements
- Must be at least 18 years of age.
- Satisfactory completion of criminal background checks and any other required screenings.
- Strong communication skills (oral and written).
- Compassionate, patient, and trustworthy demeanor.
- High school diploma or equivalent preferred.
- Valid driver’s license and good driving record preferred.
- CPR certification is a plus.
Schedule: 14.5 hours per week (spread across 3 days - M/W/F), with after morning start time.
