We’re hiring an Admissions Coordinator to manage referrals, tours, and the full admission process from first contact to move-in day.

What you’ll do
Manage incoming referrals from hospitals, physicians, and community partners
Review clinical info and verify insurance and payer eligibility
Give tours and walk families through the admission process
Coordinate admissions with nursing, therapy, social services, and business office
Complete admission paperwork accurately and on time
Build and maintain relationships with hospital case managers and referral sources
Track census and referral activity and report to leadership
What you’ll need
Admissions, case management specifically in a SNF setting.
Working knowledge of Medicare, Medicaid, and managed care
Strong communication and customer service skills
Organized, detail oriented, and comfortable with a fast pace
