Care Coordinator
Overview
Harmony Health Partners WA is seeking a dedicated and detail-oriented Care Coordinator to join our team. This role is central to helping individuals and families navigate health and social services. The Care Coordinator will coordinate care, manage schedules, and ensure that all necessary information is communicated effectively among providers, community partners, and clients. The ideal candidate is passionate about patient care, culturally responsive, and able to balance clinical knowledge with strong organizational and interpersonal skills.
Key Responsibilities
- Care Coordination & Communication
- Serve as the primary point of contact for assigned clients.
- Schedule appointments, manage follow-ups, and maintain ongoing communication between clients, families, and providers.
- Ensure care plans are implemented and updated in collaboration with the client, family, and care team.
- Care Planning & Documentation
- Complete and regularly update client care plans and assessments.
- Maintain accurate, timely progress notes and case files in compliance with HIPAA and organizational standards.
- Document interventions, services provided, and client progress.
- Client & Family Support
- Provide culturally appropriate education and coaching to improve client self-management skills.
- Engage families, caregivers, and peer supports in the care process when appropriate.
- Assist clients in navigating insurance, treatment options, and available community resources.
- Transitional & Community Care
- Support clients during hospital/nursing facility discharges, ensuring smooth transitions and timely follow-ups.
- Identify and connect clients to community and social support services (housing, food access, mental health, substance use, etc.).
- Accompany clients to critical appointments when needed.
Qualifications & Requirements
- Must meet one of the following:
- Licensed or Credentialed: RN, ARNP, LPN, PA, clinical psychologist, psychiatrist, licensed mental health counselor, marriage & family therapist, agency affiliated counselor, or certified chemical dependency professional.
- Education Pathways: Bachelor’s or Master’s degree in social work, psychology, human services, behavioral sciences, or related field.
- Alternative Qualifications: Certified Medical Assistant with an Associate’s degree or Indian Health Service (IHS) Certified Community Health Representative.
- Preferred Experience
- Experience working in a medical, dental, or behavioral health office, or with community-based care coordination.
- Strong knowledge of medical terminology and comfort with electronic health records (EHR) and case management systems.
- Demonstrated cultural humility and ability to serve diverse populations.
- Core Skills
- Excellent organizational skills; able to manage multiple clients and tasks effectively.
- Strong interpersonal and communication skills, with the ability to build trust with clients and collaborate with providers.
- Proficiency with Microsoft Office Suite, health data platforms, and case management software.
