We focus on care coordination, community connection, and empowering participants to improve their health and quality of life.Position SummaryWe are seeking a compassionate and organized Care Coordinator to join our team. This role is hybrid/remote and involves working directly with participants to assess needs, create care plans, and connect them to health and community resources. The Care Coordinator helps ensure participants receive the right support to meet their health and life goals.ResponsibilitiesConduct outreach and assist with participant enrollment.Complete health and social needs assessments to identify participant goals and barriers.Develop and update individualized, person-centered care plans.Coordinate medical, behavioral health, and community services across multiple providers.Provide education, resources, and advocacy to help participants manage chronic conditions.Track progress, update care plans, and maintain accurate documentation.Build relationships with participants, families, healthcare providers, and community partners.Attend virtual and occasional in-person team meetings and trainings.QualificationsTo qualify for this position, candidates must meet at least one of the following:Registered Nurse (RN), Nurse Practitioner (NP), Licensed Practical Nurse (LPN/LVN)Psychiatrist, Psychiatric Nurse, Physician Assistant (PA)Clinical Psychologist, Licensed Mental Health Counselor (LMHC), Agency Affiliated Counselor, or Licensed Marriage & Family Therapist (LMFT)Social Workers (MSW, BSW, or related prepared social workers)Certified Chemical Dependency Professional (CDP/ SUDP)Medical Assistants and individuals with equivalent healthcare or human services experience may also be considered with an HCA-approved exception requestOther Requirements:Strong communication, organizational, and problem-solving skillsAbility to work independently in a hybrid/remote environment while managing a caseloadKnowledge of Medicaid, Medicare, or community-based services is a plusCultural humility and ability to work with diverse populationsProficiency with Microsoft Office and case management softwareBilingual or multilingual applicants are strongly encouraged to applyBenefitsCompetitive pay (based on experience)Hybrid/remote flexibility with supportive team environmentPaid training and professional developmentCareer growth opportunities in health and human servicesHow to ApplySubmit your application and resume through ZipRecruiter today.
