Job description:
Harmony is looking for a self-motivated, results-driven, and team-oriented individual who is able to operate in a fast-paced environment. The role of the CMHW is to help children, youth and their families build a support system, bridge gaps in the service plan by making appropriate referrals and monitor the participants’ progress in following through with recommendations. The CMHW is a member of the care coordination team. This candidate must be excited to inspire, motivate, utilize communication skills, to support children and families through this program. Strong organizational skills and a positive attitude are a MUST!
Responsibilities
- Conducts outreach and engagement activities for children, youth, and families in need of services. Activities include but are not limited to street/community outreach, distributing informational materials, and peer networking.
- Conducts follow-up including home visits
- Links families with services by providing information, referrals, registrations, initial assessments, education, and linkage to a care coordinator for wraparound services as needed.
- Organizes and conducts peer educational presentations
- Assists families and individuals in accessing the service system, linking children, youth, and family members to community resources by providing cultural mediation between communities and the health and human service system
- Provides informal counseling, and social support within the family’s cultural beliefs and practices to assist them to maintain their safety, well-being, and functioning within their community.
- Provides culturally appropriate community education to promote services, advocates for individual and community needs, and assures families get the services they need.
- Participates in teamwork activities, meeting with families, care coordinators, and key stakeholders, and participates in partnership meetings, events, and training as required
- Maintain proper documentation of registrations, referrals, and assessment of needs, strengths, and resource and service plan reviews. Track participant’s progress toward service goals.
- Maintain very flexible work hours at their designated agency or community center so families to be able to access them for assistance and information.
- Completes other assignments as deemed by the Supervisor in collaboration with the Partnership leadership to attain specified outcomes.
- Performs other job-related duties and responsibilities as may be assigned from time to time and mutually agreed upon between CMHW and CMHW Supervisor.

Experience and Qualifications
- High School Diploma or GED
- Certificate of completion of Level I CMHW Training- 30 hours preferred, but not required. (May acquire while on the job)
- Completion of professional development training post-CHW Level I
- Knowledge of the community, Broward System of Care, and its community partners.
- Familiarity with community organizations serving Children, Youth, and Families
