Job Description
- Harmony Center seeks a Registered Social Worker (RSW) to add to their growing team. The Registered Social Worker (RSW) helps residents and families understand and cope with the psychosocial aspects of their lives involving change, grief, and loss through crisis intervention - individual, group, and family counseling.
Responsibilities:
- Coordinates admissions with the admissions team as appropriate
- Manages resident caseload
- Provides one-to-one and group counseling as appropriate
- Serves as a Discharge Planner
- Assists in Vulnerable Adult investigations and completes necessary documentation and reports.
Qualifications:
- Bachelor's Degree from an accredited school of social work
- Licensed by the LA State Board of Social Work
- One to three years experience in LTC
