Medicaid Intake Coordinator – Home Care Admissions
Location: Plano, TX | Full-Time | In-Office | Flexible Weekday Schedule
Harmony at Home Senior Care, a locally owned home care agency in Plano, TX, is expanding services through Medicaid Waiver programs and seeking a Medicaid Intake Coordinator to join our office team.
This role is ideal for someone with Medicaid experience who thrives in a client-facing, fast-paced environment and wants to grow into a leadership role in our admissions department. As our Medicaid program grows, this position may expand into overseeing broader intake operations, including training and supervising future intake team members.
We’re looking for someone who finds fulfillment in helping families access life-changing services during vulnerable moments. You’ll be a compassionate guide for clients navigating the Medicaid process while ensuring a smooth, professional onboarding experience.
Job Responsibilities:
Serve as the first point of contact for new Medicaid-eligible clients.
Assist families with eligibility, enrollment, and onboarding for home care services.
Coordinate all steps of the intake process, including documentation, communication with case managers, and scheduling.
Stay up to date on Medicaid Waiver requirements (e.g., STAR PLUS HCBS – a Texas Medicaid program that helps seniors and people with disabilities remain in their homes) and ensure clients remain in compliance.
Maintain accurate records and enter client information into internal software systems.
Work collaboratively with nursing, scheduling, and service coordination teams to ensure smooth transitions into care.
Communicate professionally and compassionately with clients, families, and healthcare partners.
Qualifications:
Experience with Medicaid Waiver programs is required (e.g., STAR PLUS HCBS, CHOICE, or similar).
Prior experience in home care, healthcare admissions, or case management is required.
Excellent written and verbal communication skills.
Organized, proactive, and able to handle multiple priorities at once.
Bilingual in English/Spanish is highly preferred.
Proficient with computers; experience using scheduling or CRM software is a plus.
Schedule & Pay:
Full-time, in-office position
Flexible schedule, Monday–Friday, standard business hours
No evenings, weekends, or marketing travel required
