Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance using equipment such as Hoyer lifts or resident’s
if applicable. - Monitor residents’ vital signs and observe for any changes in health or behavior, documenting findings accurately while maintaining HIPAA compliance.
- Support medication administration following care plans and ensuring proper documentation in accordance with healthcare regulations.
- Prepare nutritious meals tailored to individual dietary needs and assist residents with meal consumption, ensuring safety and proper nutrition.
- Engage residents in social activities, community outings, or recreational programs to promote socialization and mental stimulation.
- Maintain a clean and safe environment by performing light housekeeping duties such as cleaning resident areas, laundry, and general sanitation tasks.
- Follow all safety protocols during patient care activities, including CPR and First Aid procedures when necessary.
- Collaborate with healthcare professionals, social workers, or family members to develop or update personalized care plans that reflect each resident’s needs and goals.
- Use effective patient observation techniques to monitor for signs of dementia, Alzheimer’s disease, autism spectrum disorder, or other conditions requiring specialized memory or behavioral care.
- Operate equipment such as Hoyer lifts safely to prevent injury during transfers and assist with mobility challenges.
- Provide compassionate support during hospice or palliative care situations, respecting residents’ dignity and comfort at all times.
Qualifications
- Must be 18 years or older
- Must pass a state background check
- Valid driver’s license preferred
- Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred.
- Knowledge of HIPAA regulations to ensure confidentiality of patient information.
- Certifications in CPR and First Aid are required; additional certifications such as medication administration or dementia care are a plus.
- Experience working with people with developmental disabilities, autism spectrum disorder, Alzheimer’s disease or dementia is desirable.
- Ability to lift heavy objects safely and perform physically demanding tasks associated with patient transfers or mobility assistance.
- Strong observational skills for monitoring health status and behavioral changes.
- Excellent communication skills to collaborate effectively with team members and family members.
