Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Coordinate patient care plans involving discharge planning, case management, and collaboration with physicians for diagnostic evaluation and treatment adjustments.
- Provide specialized care for diverse populations including geriatrics, and those requiring memory or dementia care.
- Engage in patient education on health coaching topics including nutrition, medication administration, and chronic disease management.
Requirements
- Valid RN license in good standing with current state registration.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10)
- Excellent patient assessment skills
- Experience working with individuals with disabilities or those requiring specialized memory or dementia care is highly desirable.

Pay: $40.00 - $45.00 per hour
