Clinical Director

Harmonia Home Health
Los Angeles, CA Full Time
POSTED ON 7/9/2026
AVAILABLE BEFORE 1/4/2027
Looking for your next leadership role—but not another corporate home health job?

At Harmonia Home Health, we're building something different.

We're a concierge private-pay home health agency that provides exceptional care to our clients while fostering a collaborative, supportive environment for our team. We're growing, refining our processes, and looking for an experienced RN leader who enjoys being part of building something great—not just maintaining the status quo.

One of the biggest differences you'll notice here is that our clients pay privately for their care. That means we're able to focus on providing the care our clients truly need—not what an insurance company or Medicare will approve. While we maintain the highest clinical standards and comply with all California home health regulations, our team isn't constrained by insurance-driven visit limits, reimbursement requirements, or productivity pressures that often come with traditional home health agencies. If you became a nurse to provide exceptional patient care, you'll appreciate the difference.

This is a hands-on leadership position where you'll work closely with our Operations team, physicians, nurses, and clients. If you enjoy solving problems, mentoring staff, improving processes, and being part of a close-knit team, we'd love to meet you.

This role is probably NOT the right fit if you:
  • Are looking for a primarily remote position.
  • Prefer highly structured environments where every process is already established.
  • Prefer to work independently with minimal collaboration.
  • Aren't comfortable adapting as priorities change.
You'll probably thrive here if you:
  • Enjoy building and improving processes.
  • Stay calm under pressure.
  • Are approachable, positive, and collaborative.
  • Like solving problems instead of pointing them out.
  • Take ownership and don't wait to be told what to do.
  • Appreciate working in an office where teamwork, communication, and mutual respect matter.
  • Value providing patient-centered care instead of insurance-driven care.
  • Want your ideas to have a real impact.
What You'll DoClinical Leadership
  • Provide clinical oversight and support to our nursing team.
  • Review documentation, physician orders, and care plans for accuracy and compliance.
  • Conduct RN and LVN assessments, including occasional in-home client visits.
  • Ensure compliance with California home health regulations and HIPAA requirements.
Care Coordination
  • Collaborate with physicians, case managers, clients, and families.
  • Serve as a clinical resource for patient care concerns.
  • Partner with administrative staff to ensure accurate documentation for billing and continuity of care.
Team Leadership
  • Mentor and support nurses in clinical best practices.
  • Assist with competency validation and performance coaching.
  • Help foster a positive, accountable, and supportive clinical culture.
Process Improvement

Because we're a growing organization, we're continually improving how we work.

We're looking for someone who enjoys identifying opportunities, helping create better systems, and contributing ideas that improve both patient care and team efficiency.

Previous experience helping build or improve workflows in a home health agency is highly valued.

Staffing Support
  • Collaborate with Operations regarding staffing decisions.
  • Help ensure safe nurse assignments based on patient needs.
  • Support field staff with clinical guidance when needed.
What We're Looking ForRequired
  • Current, unrestricted California RN license.
  • Minimum 5 years of nursing experience.
  • At least 2 years in a home health leadership role (Clinical Director, Director of Nursing, Director of Patient Care Services, Clinical Supervisor, Clinical Manager, Assistant Director of Nursing, or similar).
  • Strong knowledge of California home health regulations.
  • Excellent communication and leadership skills.
  • Valid California driver's license.
  • Ability to work full-time from our Los Angeles office.
Highly Preferred
  • Experience in private-pay or concierge home health, or a desire to transition away from traditional Medicare/insurance-driven home health.
  • Experience with AlayaCare or similar EHR.
  • Experience helping build or improve operational or clinical processes.
  • Experience working in a startup, rapidly growing agency, or evolving healthcare organization.
Work Environment

This is a full-time, in-office leadership position based in Los Angeles.

You'll work closely with our Operations team every day to support nurses, solve problems, collaborate on patient care, and help shape the future of our growing agency.

Our days aren't always predictable. While this is an in-office leadership role, schedules may vary based on the needs of our patients, clinical team, and business operations. We're looking for someone who values flexibility, teamwork, and pitching in when needed.

On-Call Expectations

This leadership role may include occasional evening, weekend, or holiday on-call responsibilities depending on patient needs and operational demands. When needed, the Clinical Director serves as a clinical resource during urgent patient or operational situations.

Benefits
  • 401(k) with company match
  • Supportive leadership team
  • Opportunity to make a meaningful impact within a growing organization
We'd Love to Hear From You

Instead of sending a generic cover letter, please answer these questions:

  1. Tell us about a process or workflow you helped improve in a home health agency. What was the challenge, and what did you do?
  2. Why are you interested in joining a growing home health agency instead of a larger, more established organization?
