What You’ll Do
- Provide Health Home care management and care coordination
- Conduct community and home visits
- Develop person-centered care plans
- Connect members with medical, behavioral health, housing, and community resources
- Coordinate with providers and support systems
- Complete timely documentation and maintain compliance standards
- Bachelor’s degree in human services or related field preferred
- Experience in case management, behavioral health, social services, or community support preferred
