Overview
We are seeking a dedicated case management nurse to join our Clinical Team in their Cancer Prevention Program. This position will support the Cancer Prevention, Education, Screening, and Treatment (CPEST) program three (3) days per week, providing comprehensive nursing case management and patient navigation services.
Essential responsibilities include:
Responsibilities
1. Provide Nursing Case Management for CPEST Clients, including:
a. Completing patient intakes
b. Coordinating cancer screenings
c. Linking patients to appropriate healthcare providers
2. Maintain and Monitor Patient Records
a. Update and track patient clinical information in the CPEST
Tracking Database
b. Conduct regular patient follow-up to ensure continuity of care
3. Facilitate Communication
a. Coordinate with patients and healthcare providers regarding
covered services and needed referrals
4. Support Outreach and Enrollment Activities
a. Participate in outreach, education, recruitment, and provider
relations to expand program enrollment
b. Enter outreach activity data accurately and in a timely manner
Pay: $40.00 - $45.00 per hour
Expected hours: No more than 24.0 per week
