Job Summary
We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will coordinate patient care across various settings, ensuring seamless transitions from hospital to post-acute care and outpatient services. This role requires a strong foundation in acute and chronic care management, with an emphasis on patient-centered approaches, utilizing extensive clinical knowledge to optimize health outcomes. The RN Case Manager will collaborate with multidisciplinary teams, manage complex cases, and facilitate discharge planning to promote efficient and effective patient recovery.
Duties
- Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations.
- Develop and implement individualized care plans in collaboration with physicians, specialists, and other healthcare providers.
- Coordinate inpatient and outpatient services, ensuring timely discharge planning and post-acute care arrangements.
- Manage utilization review and case management activities to optimize resource use while maintaining high-quality patient care.
- Facilitate diagnostic evaluations, monitor patient progress, and adjust care plans as necessary.
- Maintain detailed medical records in compliance with HIPAA regulations and hospital policies.
- Provide education to patients and families regarding treatment options, medication management, and post-discharge instructions.
- Collaborate with hospital medicine, emergency medicine, ICU teams, behavioral health specialists, and other departments to ensure comprehensive care delivery.
- Engage in patient service activities that enhance the overall patient experience within hospital or outpatient settings.
- Support infection control protocols and participate in quality improvement initiatives related to acute care, trauma centers, or specialized units such as PICU or Geriatrics.
Qualifications
- Valid RN license with current registration; experience in critical care settings preferred.
- Proven experience in Level I Trauma Centers or Level II Trauma Center environments is highly desirable.
- Extensive background in ICU, emergency room, medical-surgical units, or post-acute care settings such as inpatient or outpatient clinics.
- Strong knowledge of physiology, medical terminology, HIPAA compliance, and diagnostic evaluation procedures.
- Demonstrated expertise in discharge planning, utilization management, case management, and managed care processes.
- Experience with patient assessment techniques including vital signs monitoring and clinical documentation.
- Familiarity with hospital records systems and medical records management.
- Excellent communication skills for effective patient service delivery and interdisciplinary collaboration.
- Ability to handle complex cases involving infectious disease care, behavioral health issues, geriatrics, pediatrics (PICU), or primary care populations is a plus.
