The Opportunity

We are currently seeking a dedicated Admissions Nurse - RN/LPN

What You Will Do

• Conduct comprehensive assessments of residents' medical needs and develop individualized care plans

• Monitor residents' progress and make necessary adjustments to their care plans

• Provide education and support to residents and their families regarding their medical conditions and treatment options

• Collaborate with insurance providers to obtain necessary authorizations for medical procedures or treatments

• Maintain accurate and up-to-date documentation of resident interactions and care plans

• Duties as assigned

What You Must Have

• Active RN/LPN license in good standing in the state of practice.

• Experience in case management or care coordination is preferred but not required
