Harborview Medical Center AACS Population Health has an outstanding opportunity for a Registered Nurse (RN) Care Manager Care Manager & Diabetes Resource Nurse!
WORK SCHEDULE
- 100% FTE, 40 hours/weekly

PRIMARY JOB RESPONSIBILITIES
- Ongoing, proactive care management for high-risk, medically complex patients with primary focus on diabetes
- Use the nursing process of assessment, diagnosis, care planning, implementation, and evaluation to provide safe, therapeutic, and coordinated care for primary care patients during in-person and virtual interactions
- Support patient care transitions from recent hospital or emergency department discharges, specialty to primary care, and reduces barriers to care
- Partner with patients and families to develop individualized care plans, support disease self-management, and improve knowledge of their health conditions. Regular collaboration with providers, pharmacists, hospital and clinic personnel, and diabetes experts ensures appropriate access to services, efficient use of resources, and high-quality care
REQUIREMENTS
- Current license to practice as a Registered Nurse in the State of Washington
- At least two years of RN ambulatory care /outpatient experience or care management experience
- Certification as a Diabetes Care & Education Specialist, must be obtained within 6 months of hire
