Our patients are seen across diverse settings — private residences, skilled nursing facilities (SNFs), board-and-care homes, and assisted living communities.
Position Summary
We are seeking a Nurse Practitioner (NP) with a passion for wound management and patient-centered care. The ideal candidate is reliable, compassionate, and able to work independently while maintaining strong communication and coordination with our interdisciplinary team.
Key Responsibilities
- Provide wound assessment, treatment, and follow-up in home and facility settings
- Communicate and coordinate care plans with physicians, facility staff, and caregivers
- Maintain accurate and timely documentation in the EMR
- Collaborate with case managers and other healthcare professionals to ensure continuity of care
- Drive between patient locations within the assigned region
Qualifications
- Current and active California Nurse Practitioner license (FNP, AGNP, or equivalent)
- Reliable transportation and willingness to travel between patient locations
- Strong clinical assessment and bedside skills
- Excellent verbal and written communication abilities
- Comfortable using electronic medical record (EMR) systems
- New graduates are welcome – must be eager to learn, coachable, and committed to professional growth
Preferred Qualifications
- Experience in wound care, home health, hospice, or post-acute settings (preferred but not required)
- Wound care certification (WCC, CWCN, CWS, or similar) a plus
