Harbor Health & Wellness is seeking a warm, reliable Nurse Practitioner to provide high-quality primary care to seniors through on-site visits within low-income apartment buildings and senior housing communities. This flexible, low-stress role is ideal for NPs looking for part-time work, supplemental income, or a stable weekly schedule with autonomy.
Responsibilities
- Conduct on-site visits & monthly follow-ups
- Perform Annual Wellness Visits (AWVs) and sick visits
- Manage chronic conditions and medications
- Provide preventive care, patient education, and health coaching
- Order labs, imaging, and coordinate care with specialists
- Document visits accurately in the EMR
Requirements
- Active NY NP license (FNP/AGNP/ANP)
- Board certified; NPI & DEA (or ability to obtain)
- Experience in primary care or geriatrics preferred
- Strong clinical judgment & ability to work independently
