To help further the mission of Harbor, we are looking for an extremely talented Lead Dietitian to join our team at the Daniel Driscoll Neponset Health Center in Dorchester.
Role:
The Lead Registered Dietitian (RD) will be responsible for the programmatic supervision of the nutrition team, guiding and mentoring fellow RDs, ensuring optimal patient care, and enhancing the overall nutrition education across Harbor Health clinics. The Lead RD will play a key role in training, providing support, and serving as a liaison between the nutrition team and other departments. The Lead RD will also manage a patient caseload, administering optimal nutrition care to health center patients and families through assessing nutrition needs and developing and implementing individualized, evidence based care plans. This is a Full Time/Hybrid position.

Responsibilities:
- Provides medical nutrition therapy, nutrition education and food access resources to a caseload of patients from HHSI’s 4 community health centers.
- Assists in improving patient metrics such as A1C, weight, and cholesterol levels, contributing to better health outcomes and reduced costs of care for patients managing diabetes and other nutrition-related conditions.
- Provides medical nutrition therapy and nutrition education to Ryan White Program-enrolled patients living with HIV.
- Attends program-related clinical care review meetings as needed and with scheduling notice to enable clinical collaboration on patient cases.
- Mentors & supervises RDs during onboarding and throughout their careers at Harbor Health.
- Identifies and addresses knowledge gaps within the RD team, providing training, guidance, and resources to improve their skills.
- Creates and compiles educational materials for RDs to use during patient appointments.
- Ensures that other healthcare providers have the necessary resources to supply patients with comprehensive nutrition education.
- Leads monthly nutrition meetings to facilitate discussions around complex patient cases, areas of uncertainty, and questions within the nutrition department.
- Presents case studies to help expand RDs’ clinical knowledge and ensures the continuous development of their care practices.
- Represents the RD team in meetings with other departments, serving as the liaison between the nutrition team and other teams across Harbor Health.
- Actively communicates and addresses any needs or challenges from the RD team to ensure collaboration and effective patient care.
- Bachelor’s degree in Nutrition, Masters preferred
- Registered Dietitian within the Academy of Nutrition and Dietetics and current MA Dietitian/Nutritionist licensure required
- Current BLS (Basic Life Support for Healthcare Providers) Certification required
- Certified Diabetes Care and Education Specialist (CDCES) highly preferred
- Extensive experience in clinical nutrition, with a focus on diabetes care and education preferred
- Proven leadership or supervisory experience, with the ability to mentor, educate, and guide other RDs required
- Strong communication and interpersonal skills to work collaboratively with internal teams and departments.
- Knowledge of current nutrition best practices, evidence-based guidelines, and the latest research in the field
- Ability to handle complex patient cases and provide expert-level nutrition counseling
- Experience in creating educational materials for patients and healthcare providers
- Familiarity with electronic health record (EHR) systems and documentation, EPIC/OCHIN desired Ability to present and facilitate learning sessions or case study discussions
- Demonstrated ability to improve patient outcomes through nutrition interventions
- Experience with Office 365 including Word, Excel, Outlook and Teams
- Some travel between Harbor Health Services, Inc. sites is required; must be able to travel to other sites in a timely manner.
