Working alongside a multidisciplinary team, you will help promote health, prevent illness, manage chronic conditions, and support patients in accessing the care and resources they need.

Reporting to the Nurse Manager, you will provide direct patient care, support providers, serve as a clinical resource for Certified Medical Assistants, and help ensure continuity of care across Harbor Care programs and community partners.

In This Role, You Will
- Provide patient-centered nursing care in a primary care environment.
- Advocate for patients through assessment, clinical judgment, and care coordination.
- Communicate patient needs and concerns respectfully with providers and care team members.
- Administer medications, including IM and SC medications, and provide patient education as needed.
- Perform blood draws and laboratory processing as necessary.
- Monitor patients with chronic or long-term health conditions.
- Support individuals and families through education, care planning, and connection to community resources.
- Participate in quality assurance activities and interdisciplinary meetings.
- Serve as a clinical resource for CMAs and support nursing student education as assigned.
- Help monitor vaccine supply and support required State and Federal reporting.
- Maintain patient privacy, confidentiality, and compliance with HIPAA standards.
- Promote Harbor Care’s mission, values, and commitment to integrated, compassionate care.
Required Qualifications
- Active NH Registered Nurse licensure.
- Strong clinical nursing skills, including vital signs, patient assessment, documentation, and follow-through.
- Excellent oral, written, and reading communication skills.
- Ability to understand and execute verbal and written instructions.
- Ability to function effectively in a team-oriented environment.
- Demonstrated resourcefulness, flexibility, professionalism, and integrity.
- Respect for people of diverse cultural, religious, ethnic, and personal backgrounds.
- Ability to organize, prioritize, and complete work with attention to detail.
- Commitment to maintaining patient privacy and confidentiality in accordance with HIPAA requirements.
- Bachelor’s degree preferred, but not required.
- Experience working in a Federally Qualified Health Center.
- Experience in a community healthcare setting.
- Experience with substance use treatment and medication-assisted treatment practices.
- Knowledge of Microsoft Office and Centricity/Athena EMR software.
- Bilingual Spanish/English preferred, but not required.
- Maintain a neat, clean, and professional workspace.
- Adhere to Harbor Care’s dress code, including scrubs and appropriate footwear.
- Complete required paperwork and reporting in a timely manner.
- Participate in training for Harbor Care staff, residents, and patients as needed.
- Complete other duties as assigned by the Nurse Manager.
