Harbor is seeking an Older Adult Wellness & Behavioral Health Prevention Provider to join the team in Toledo! This role supports the implementation of a holistic, prevention-focused behavioral health initiative through leading program operations, facilitating workshops, providing individualized coaching, building community partnerships, and coordinating public awareness efforts to help ensure reduced risk factors, strengthened protective factors, and improved mental wellness among older adults.
Position is full-time.
REQUIREMENTS
Education:
- Bachelor’s degree in Social Work, Public Health, Gerontology, Psychology, or a related field.
Experience:
- 3 years of experience in community-based behavioral health, prevention, or aging services.
- Strong understanding of SAMHSA’s Strategic Prevention Framework and prevention science.
- Experience working with older adults, caregivers, and/or peer support models.
- Proven excellent facilitation, communication, and interpersonal skills.
- Experience working collaboratively with multidisciplinary teams and community stakeholders.
- Cultural humility and experience in engaging diverse populations.
- Proficiency with Microsoft Office and basic data tracking systems.
- Ability to work flexible hours, including occasional evenings or weekends for events or workshops.
- Strong written and verbal communication skills are necessary.
Licensure and Certifications:
- Must currently hold or obtain a Registered Applicant status within 90 days of hire.
- OCPSA required to be obtained within 1-year of the date Registered Applicant status is obtained.
- Ohio Certified Prevention Specialist (OCPS) certification required within 1-year of the date OCPSA is obtained.
- CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
- Must be proficient and accurate in computer use, including Microsoft Word.
- When applicable, non-licensed staff will be required to complete 20 hours of CEUs every 24 months
- Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
Preferred Qualifications
- LSW or OCPS.
- Understanding in childcare center support and appropriate curriculum and development.
- Knowledge of Ohio Prevention systems.
RESPONSIBILITIES
Essential Functions and Primary Responsibilities:
- Plan, implement, and evaluate prevention-based programming aligned with SAMHSA's prevention strategies.
- Deliver evidence-informed educational workshops and coaching sessions on mental health, substance misuse prevention, stress management, and healthy aging.
- Coordinate peer-led support groups and caregiver education sessions.
- Ensure programming is culturally responsive and adapted to the diverse needs of older adult populations.
- Build relationships with local behavioral health providers, aging services, wellness programs, and faith-based or cultural organizations.
- Create linkages to age-appropriate treatment, social supports, and community wellness resources.
- Collaborate with the Local County Mental Health & Recovery Services Board (LCMHRSB) to refine goals, metrics, and strategic partnerships.
- Support public awareness campaigns to reduce stigma and promote behavioral health help-seeking among older adults and caregivers.
- Assist in data collection and outcome tracking aligned with the Strategic Prevention Framework.
- Monitor program impact, participant engagement, and satisfaction to inform quality improvement.
- Prepare reports for internal leadership and funding stakeholders as required.
- Develop outreach materials and facilitate participant recruitment efforts.
- Maintain accurate program records, scheduling, and documentation.
- Attend relevant trainings and participate in community coalitions or advisory groups focused on aging and behavioral health.
